COVERAGE FOR SUBSCRIBERS UNDER 19 YEARS OF AGE IS AVAILABLE FOR ELIGIBLE RESIDENTS OF CA. APPLICATIONS MUST BE SUBMITTED DURING THE INITIAL OPEN ENROLLMENT PERIOD BETWEEN 1/1/11 AND 3/1/11, DURING THE ANNUAL OPEN ENROLLMENT PERIOD SUBSEQUENT TO THE INITIAL OPEN ENROLLMENT PERIOD THAT IS THE MONTH OF THE CHILD'S BIRTH DATE, OR WITHIN 63 DAYS OF THE OCCURRENCE OF CERTAIN QUALIFYING EVENTS. PLEASE CALL 866-694-3258 FOR MORE INFORMATION ON QUALIFYING EVENTS OR TO APPLY FOR COVERAGE.
